Report of coroner's findings from suicide case in MTKDC
Dear All registries and administrative offices of courts and tribunals for which the Judiciary Administrator has responsibility,
I am writing to make an information access request regarding a report of Coroner’s findings after an inquest hearing.
The hearing was regarding a suicide case in Ma Tau Kok Detention Centre in 2019.
The case reference number is KCRN 19017855.

The first day of the hearing was Sep 6, 2022.
I would be grateful if a report of the Coroner’s findings, or any other relevant materials from or regarding the proceedings, could be provided.

Information (“the Code”), the Code does not apply to information held by
courts, tribunals or inquiries.

Thank you very much for the information you provided.
I noticed that the form you shared requires an authorization letter from the deceased. However, as far as I understand, the deceased in this case had no family in Hong Kong. Could you please kindly advise how to request the findings in this case?

Dear Sir / Madam,
Thank you for your email of 30.9.2022.
Regarding your request for the relevant court records of
the Coroner’s Court, please be informed that the purpose of the
authorization letter mentioned in our previous reply of 27.9.2022 is to
evince that the application is made by an authorized person who is duly
assigned by the direct family member of the deceased. The authorization
letter should be submitted along with a copy of the identity document of
the direct family member of the deceased.
If the authorization letter is not available, you may fill
in the application form and set out your relationship with the deceased,
by what means you have attempted to contact the direct family member of
the deceased and the reasons of your application. Please also provide
your contact details for the reply of the Coroner’s Court.
