I would like to request the blank or specimen questionnaires (both English and Chinese, in PDF format) of the surveys as listed in Annex D - Surveys Conducted in 2015-2016 under Enacted Census and Statistics Orders, and Annex E - Voluntary Surveys Conducted in 2015-2016, in your publication An Outline of Statistical Development 2016 Edition. The period should be from 1981 to 2016.
