Dear Office of The Ombudsman

I would like to have the following information pursuant to the Code on Access to Information, specifically of complaints regarding Code on Access to Information requests made to your department:

1. The total number of complaints received and concluded by the Ombudsman concerning alleged non-compliance with the Code
2. The number of concluded cases where a government department or public body was explicitly found to be in breach of the Code or had not fully complied with the provisions of the Code
3. The number of times the Ombudsman had issued formal recommendations advising a department or other body to disclose or re-examine the release of withheld information following a finding of non-compliance (in accordance with the Code)
4. The number of individual times that the departments or bodies refused to act upon the Ombudsman's recommendations on the respective Code on Access to Information complaints
5. The number of times the Ombudsman had submitted a report to the Chief Executive under the Ombudsman Ordinance due to a department's failure or refusal to implement a recommendation regarding an Access to Information complaint

Please provide me with these statistics dating as far back as such records have been made. I am happy to take the numbers in any aggregated form but I would be grateful if you could present them in PDF or spreadsheet rather than tabular format as I cannot read it properly.
