Q1. Which community isolation facilities are operated, managed, or
supervised by you?
Q2. For each and every community isolation facility listed in Q1 above,
how many Registered Medical Practitioners, Registered Nurses and Enrolled
Nurses perform duties each day since opening?
Q3. (a)                 How many service users requested for medical
assistance in each and every community isolation facility listed in Q1
since opening?
(b)                 How many medical assistance requests were entertained
for each and every community isolation facility listed in Q1? Please
explain the kind of medical assistance that were provided by Registered
Medical Practitioners, Registered Nurses and Enrolled Nurses.
(c)                 How many medical assistance requests were denied for
each and every community isolation facility listed in Q1? Please provide
reasons for denial.
Q4. For each and every community isolation facility listed in Q1, how many
service users were transferred to hospitals since opening?
